Registration
This line 260 9227 is open between 8:30am to 1.00pm & 2.00pm to 5.50pm for information on how to register.
You will be asked if you are staying in the practice area (you will be asked your address) and if you plan to be in the area for more than 3 months. (less than 3 months you may be seen as a temporary patient).
There are two GP practices in the Health Centre who register patients on alternate days. If you are staying with a family member or partner you should register with the same practice as them.
You will be asked to attend the Practice between 9:00am and 12.00 noon or 2.30pm and 5.15pm on the day the Green Practice is registering, bringing with you the following:

New patients arriving at reception with the appropriate documents will be given a registration form and a health questionnaire to complete. To speed up this process, you may complete both registration forms shown below, bringing these along with the above confirmation documents.
NB:- Posted forms will NOT be actioned and must be printed double-sided, please bring all forms and documents in person.

All new patients are invited to attend for a ‘New Patient Check’ with our one of our Nurses. This is a medical check-up which covers:
blood pressure, height, weight, urinalysis, family and self history of heart disease
Health promotion issues: alcohol,smoking,diet,exercise, rubella status and Testicular Self Exam (men only).
